Preventive Health Care Plans for Cats

We have 4 different Preventive Health Care Plans for Cats: Kittens, Basic Adult, Adult with Oral Diagnostic and Treatment Procedure, and Senior/Special Needs Cats. "Special Needs" cats are ones that need more frequent examinations or more detailed monitoring due to on-going illnesses occuring before they are over 10 years of age, which is the general recommendation for Senior Testing. Below is a listing of the services included in each plan. To enroll, print and complete the agreement (click on Preventive Care Plan Agreement at left), then bring to your next appointment or contact our office at 574-654-3129.   There is a $50 enrollment fee but this is waived in successive years if membership is renewed without lapse.   To avoid this fee, simply pre-pay for the entire plan in advance at the time of enrollment. For more description, be sure to read the main Preventive Health Care Plan tab at left.

Preventive Health Care Plan for Kittens

Annual Services include:

  3 Comprehensive Physical Examinations

  2 Technician Consultation/Exams

  1 Progress Examination

Vaccinations:

   1 Rabies Vaccine

   4 Kitten Distemper/Upper Respiratory Vaccination series and series final (Panleukopenia, Rhinotracheitis, Calicivirus)

   2 Feline Leukemia Vaccine series and series final

Lab Work:

   1 Feline Leukemia/Feline Immunodeficiency Virus test

   3 Intestinal Parasite Exams (Microscopic Fecal Analysis)

    1 Ear Swab/Cytology for ear mites

Parasite Control:

   3 Intestinal Deworming Treatments

   12 months Heartworm & Flea Prevention (also prevents most intestinal worms, ticks, and earmites) with Revolution. Some substitutions permitted.

   1 Ear mite treatment (Milbemite)

Surgical Procedure:

   Preferred Plan Spay or Neuter including IV fluids, multiple types of pain management, therapy laser treatment, and advanced monitoring and supportive care

   10% discount for surgical laser use and pre-operative CBC, blood profile, and clotting tests, 10% off declaw upgrade which automatically includes surgical laser usage

Additional Services:

   Microchip Permanent Identification with lifetime registration

   4 Nail Trims

10% discount on all products and services not included in the plan

  2 Intestinal Deworming Treatments

  12 months Heartworm & Flea Prevention (also prevents most intestinal worms, ticks and ear mites) with Revolution. Some substitutions permitted.

Surgical Procedures:

  Oral Diagnostic and Therapeutic Procedure including anesthesia with IV fluids, monitoring, hospitalization, ultrasonic and manual dental scaling, polishing, fluoride treatment, and Oravet Barrier Sealant

  Full Mouth X-rays (and there is no further cost if additional x-rays are taken at the same visit)

  10% discount on dental therapy such as surgery, extractions, pain medications, antibiotics, and home care products including TD

  1 Feline Leukemia Vaccine

Lab Work:

  2 Intestinal Parasite Exams (Microscopic Fecal Analysis)

  1 Early Detection Profile with Electrolytes with CBC (Red, White and Platelet cell counts), General profile (blood sugar, liver, kidney, protein, electrolyte, pancreas, and other tests),  Thyroid tests

  1 Feline Leukemia Test and Feline Immunodeficiency Test

  1 Complete Urinalysis with sedimentation exam and sterile collection

  1 Urine Protein:Creatinine Ratio

  1 Urine Culture Screen

  1 Laboratory Collection Fee

Parasite Control:

  2 Intestinal Deworming Treatments

*Appointments for examinations must be made at least 48 hours in advance, in regular time slots (same day emergency appointments are not covered). Missed appointments without 48 hours advance notice of cancellation, including for drop-off services, will be billed as though they were provided.

**No discounts are given during those years when multi-year vaccines are not due.

Sorry, but due to the extent of the discount, additional discounts for multiple pet or senior citizens are not available. No refunds for unused products or services. Discounts are only available for products used for the pet enrolled in the plan.
